I am trying create a form that prompts for a user name and a password when my database is opened. Can somebody help me with this?

I believe the database needs to be setup with security first. Once you have locked the db under options, you can setup security and then setup users with individual access to certian forms, etc...

If you're not very familiar with Access, the User Level Security Wizard under the Tools menu bar will walk you through creating a user groups and access to your tables, queries, and reports. You can then make further additions and changes through Tools>Security>User groups and Permissions.
