Multi Field Combo search Form

Hi Within the database below I have tried to create a multifield search form, however it only works when you use one of the drop down fields only,

For example If i select home in match type it show all the home results, However what i would like it to do is the following

As well as working by themselves to find the data, I need the combo boxes to work together when you press the run query/search button. This will then either run the query and produce report or run just a query. maybe an and parameter would be better suited for this, but if someone could be kind enough to take a look I would be most grateful
