Thanks Bob, I had to go to a meeting. I noticed a few things that were probably not needed for her application or that needed to be changed (like the record source query) but were there in the original template but did not see that part.

May be good to get another set of eyes on this. The main thing I see that could be causing the issue is the "Open" field. It is not populating like the original template. I'm running low on time to look further but the macros all look identical with the proper replacements for form names...

Do you see at the top where it says "Certain content has been disabled"? Click options and tell it to enable the content and see if it works then. Also, which field button are you referring to?

Do the other time periods still work?

Can you try putting the field names back to how they were before to see if that fixes the issue? What field names did you change, the labels or the actual controls? If you can give any more detail on the issue it will be a lot easier to help out.

Not sure what the bump rules/recommendations are but it seems my post is the only one lately there hasn't been at least 1 response to. I know its a weird issue, hopefully someone has experienced this before and found a solution. Thanks in advance.

From what I read I'm having a hard time finding an issue with the query other than you should really look into using table aliases. That table name is massive lol. Does the query work directly?

I have this same issue, along with another that hasn't got a response yet. Overworked, if I understand you correctly those blank columns are just there, kind of like they would be in Excel. You have no data in them, they are not essential to your table at all, they're just flat out empty...
