I know that perfection takes time but there is a lot of static information to show on the report and the layout has to be spot on as it's an official document. My main problem with recreating the document is getting it to look like the original.

I thought about recreating it but thought it would take too long to do. Using the document as a background? Is that like a watermark type thing? If so how do I do that? Would I then be able to get the gatabase to produce the correct results in the correct place?

I am trying to create a database which allows me to to record the football (soccer) matches which I referee. If a player should be cautioned or sent off during a match, then a disciplinary report has to be submitted to the appropriate Association. This report is currently an MS Word document...
