Opening a form for a specific record (1 Viewer)

Opening a form for a specific record but with 2 criteria

I use the wizard to open a form to show a dataset with specific matching criteria ie a matching field

And I note the embedded macro that is created

However I would like to open a form for a specific dataset where there is more than one criteria eg "PeriodID" and "InstitutionID"

The wizard only seems to give you an option of one matching field

The embedded macro may be able to be modified to include a second matching field but I do not know what the code would be

A copy of the wizard options (for one matching field) is attached
A copy of the embedded macro (for one matching field) is also attached

Is it possible to have a form open that matches more than one field (other than having them both as criteria in the query for the form) ?
