I am trying to extract data from a survey that is buried in a patient note. The portion I am attempting to utilize is attached below. The database is normalized. Question 1 has a fixed ID of 52 with possible Answer IDs of 12, 15, 34, 49, and 75, for example. So when it is saved, CompletedSurveyID 123 has AnswerID 12 for QuestionID 52 on SurveyID 2. Of course, the text of the question and answer is all the user sees...the database is set up for direct entry, but the decision was made to make the survey part of the patient's electronic chart. To prevent duplicating effort (since some of the surveys are quite long) I would like to import this data...probably by pasting it to a text file and importing it (unless someone has a better idea).
Can anyone recommend a good plan to capture this data? The text file looks like this:
