calling multiple fields from a query

Hi All

I have a report where I am trying to pull an employee and all related info in a query between selected dates.
I am using one combobox for the name, a second for the start date and a third for the end date,
SURPRISE---I can't make it work.
What would be the best way to proceed?

after the changes, the report calls all info from the query, not just the specific employee- I get a 5000 page report. Any ideas to narrow the search to employee specific records?
