Can anybdy help me- query on form fields value

Pls help me ...My database is attached with this mail.
I want to get results if i fill 'batch-id' =60918 and Th-No =9 in the form.

In 'Display Button' , i am running a macro which is running a simple query on data entered in form fields(Batch-id and TH-no).

Pls help me to get results.
I'll be highly obliged.

I checked file sent by you. When i click 'Run-Query button' It is not giving me empty result screen altohugh cretiera entered by me matches some records in the table.

Pls check it once more.

When I write criteria "Batch-id" = 60918 and "Th-no" = 9,
I get two records (Simi and Manish).
