Chart on form

I AM IN A HURRY!!! FRIDAY IS MY LAST DAY!!!!!

I am trying to get a chart to work on a form,

I want the 'Results' to be the data shown (lines),
over the date range (bottom),
with value range (left)

I can't get it to do this though...

can someone please help....
