Combo box Query Prompt? very stupid question?

how do I make my query look like this [ NOT a cascading sequence. completely independent of one another ]

Select Product ---> drop down combo box type list gives me options
Select Type-----> combo box options given
Select Job number--->...
Select.. blah blah blah..--->

Then I can have different options and the option to leave things blank or clear all entries so far with one button..

Also. How do I attach a form record for each record in the final query recordset?

I'm so stuck. Please help
