Corrupted dll file i think...?

Windows Vista
Access 2007

Yesterday I put together a small accdb file. On my last form I clicked the subform wizard and i get this thrown at me (see attachment). I had been using the subform wizard on a couple other forms and worked fine. I opened up a old 2003 file of mine to look at some work as reference. When i reopened the new project and attempted the subform i get the error.

I've tried several things, in no particular order...
1) repair MS Office
2) uninstall and reinstall MS Office
3) registry scans
4) attempted to make subform in blank database and run subform wizard
5) attempted to import existing db to new blank db and run subform wizard

I've done lots of googling around and looking at through threads on this site and have found similar issues related to the error 2189 but no real solution...

I'm intrigued with the last line of the error: "Before you reinstall 'project name' delete the Windows Registry keys for the 'project name' Utility Add-in and acWzlib." I'd love to do this but messing with the registry isn't something i've done before. Is there anyone that has experienced a similar issue? I've seen posts about command buttons and other wizard trouble.

I can make the subform manually using master/child field properties...

I appreciate any hints and help with this one

I'm pretty confident that I have solved this issue. I had to remove office, then remove objects from the registry. For those people frustrated with this issue in the future i've attached my notes that i used to solve the problem.
