Creating a search inside a form that filters results. (1 Viewer)

This sounds very complicated and for that i apologize, ill try and explain as best i can.

What i want is for a form to have drop down and text boxes that when entered and the search button is clicked will filter the results and show them either in a table or as a form or something similair

the 5 search criterias i have are:

Name... (which is a combobox)
Between Date (textbox) & Date (Textbox)
Quote number (Combobox)
Delivery number (Textbox)

i would like if any are filled in it will filter accordingly

Note: all the information is stored on a table called "VisitSheetTable"
