Date and Time

We would like to see this field as; Oct 25, 2012, 05:38 PM

I have tried;
Data type = date/time
With Format set as shown below

MMM DD, YYYY, hh:nn ampm
mmm dd, yyyy, hh:nn ampm
mmm dd", "yyyy hh:nn ampm

But no success.

Can I receive a suggestion on the proper formating

You just missed the / between ampm so it should be

mmm dd", "yyyy", "hh:nn am/pm

If you want the pm capitalised to PM, use ampm (or AM/PM)
