Derive Tables Problem

Hi all,
I am given an Excel Sheet, to make an entry form for that, So it is so complicated, mean I could not bring their Fields (Columns) in one single row, So Decided to make this in M.S Access, But I cant derived the Tables

1. Could u plz kindly help me how to derive the tables of that Sheet with good normalization and relation in M.S Access?
OR
2. Just Design in which I have all the fields and columns in one single row in M.S Excel?

I have recently completed a project similar to yours. Please find attached the Excel file used in the project. This file is used as the import source into an Access database for data analysis.
