Do I have to create a table before I can create a form?

Not necessarily. You can create the form first if you like, but having the Field List available and being able to set your code and references correctly from the get-go is usually a plus.

Now, that being said, unbound forms that don't tie into table data at all are often useful. Instead of going through the wizard just go to New Form, Design View.
