Fields not showing on form

I have created a form off a query. I made the form for adding new records only. When i bring up the form it is blank, the labels and fields don't even show. Anyone know why this is?

If there are no records in your table to show then your form will be completely blank.

I would have said if there's meant to be anything in your table then something is wrong with your relationships, and your links need re-doing, otherwise i agree jkl0 should be blank if its for adding new records only? why ya need a query for that tho?
