    Go for the default (light) theme, Shades of Grey or Shades of Blue. I just added the Blue one.

This is a nice example of using Google forms together with a Google sheet. Also Demonstrated is an "Array Function" Which can replace the need for having a formula for each row. For example you may have situation where a function in Column "C" Adds the value in column "A" to the adjacent value in column "B" and places the "SUM" in column "C" a formula in Each Cell of "C". This Video shows you how to produce an "Array Function" which does the calculation of all the values in ONE GO! Very useful and alludes to a technique I have yet to understand.
