How to search in a form by fields?

Hi
I have a problem that should create three search fields with check box that if checked its field may considered in search.Such as Code , Name and last name.please help me I'm confused.

Try to do this via FILTER BY FORM, CLEAR GRID, APPLY FILETER and REMOVE FILETER icons.
