if this field empty don't display record

Hello all. I have a question. Basically I have a table with 15 different fields and I want a report to kick out 5 fields based on the information contained. For instance, I have over 150 last names and when I make the report I of course have all 150 but I only want the names of the people that have the other field or fields filled in.
