This is a sample lookup input form. The calling functions takes the same type of input as a combo box. The sample form has 3 buttons opening the form with 2 column value list, a single column query string and a single column saved query. The query list all the reports. The 3 reports are empty. I stripped a production form down removing query parameters and ADO record set options.
