Managing Checkboxes - SQL append and insert (1 Viewer)

I have a form with 5 unbound checkboxes (chk_1, chk_2, chk_3, chk_4, chk_5)
I want to click on the checkbox, then click a a save button (On click event btn_save.Visble = True) then to insert the value (0 (unchecked) 1 (checked) into the tbl_data.Chk1, chk2 etc
