Multiple selection value in reports

I have a problem getting all the selected (True) check box into report under one column for each customer name

That would be because your database is not designed properly. You currently have a non-normalized design (repeating fields) and that will only frustrate you as you try to get information back out of the database for reporting.

For each customer name i have to select some check boxes like ( w_1 - w_2 - w_5 etc ) only one time selection for each customer .

i thing i am going to the dark path
