Ive been given a task, and wondered whats the best way to go about it, the form as an assessment which takes place regular, so one person can have many assassments. I would need to create a database which has one persons details then underneath using a form enter the assessment details for each assessment, eg John Smith may have been assessed 4 times in a year ill need to show this on the form, ie John Smith and his details at the top and the assessments below, 1 page at a time, sorry its vague but it seems very simple, any tips!

NameID(Autonumber)
Surname
Forname
Job Title

table 2 I have all the questions for each assessment

NameID
QuestionsID (Autonumber)
Q1
Q2
Q3
Q4 etc ti Q20

How do I get it to link tables, so i can enter multiple assessments for each name!
 
Client table with AutoNumber as PrimaryKey and Assessments table with the Client PK as a ForeignKey. The MainForm with query of Client as RecordSource with a SubForm that has a query of the Assessments table as RecordSource and using the ClientID field as the LinkChild/MasterFields.
 
The field NameID in both tables should be used to link them in a OnetoMany relationship.

Table 2 not normalized. Make each Q a different record.
 
If you use the design I suggested (MainForm/SubForm) then the ForeignKey will be handled by Access for you.
 
Cheers I think Ive sussed it from the advice, it shows what i want anyway. One more thing it may be straight forward how do I restart my autonumber so it starts at 1 again, i was testing the data, and it keeps going up, when i delete all records it doesnt start at 0.
 
Do a Compact and Repair AFTER you have deleted all records in the table, that will reset the autonumber
