Question Normalised Questionnaire database vs Data entry form

Dear all,
I have a small questionnaire for which i want to design a database. I have attached what i think is a normalised design based on the given questonnaire. My problem is that i can not figure out how to design a data entry form that looks like the questionnaire from such a normalised structure. Data Entry people usually want the data entry screen to look exactly like the questionnaire. I have attached both the questionnaire and the database.
Any thoughts on the same? Is the design ok?

• 
  free resources on how to design databases for questionnaires are not easy to come by online.
  I would appreciate any online resource that specifically deals with questionnaire database design
