Printing Field Names only if "Yes" is selected in a Y/N combo box

hello
i am trying hard to produce a report that gives me the actual name of a field if the selected option is "yes" using a y/n combo box.

I have 12 different fields with y/n options and i dont want to print every field name in the header as well
Just the actual name of the field in the content area if it is a "YES" and not printed if it is a 'No'

I hope some one can help me on this
