PW Turned in Query

I have 2 tables that I wish to get information out of, but I am unsure how to set up my query. I hope one of you can assist me.

Intake
Intake ID
Date Of Intake(Date/Time)
Caseworker(Text)

Risk Assessments
Intake ID
Substantiation Risk Level(Text)
Substantiation Risk Complete(Yes/No)
3 Month Risk Level(Text)
3 Month Paperwork Turned In(Yes/No)
6 Month Risk Level(Text)
6 Month Paperwork Turned In(Yes/No)
.....The list goes on but I just want to use these fields for now.

If a Risk Level is not null (Substantiation Risk Level, 3 Month Risk Level, 6 mo....etc.), I want to see if the corresponding paperwork has been turned in (or for Substantation Risk Level, I want to check if Substantation Risk Complete is checked).

In a report I want it to look something like this.....

Caseworker
Subst. Risk Level/PW Turned in: 23
3 Mo. Risk Level/PW Turned in: 40
6 Mo. Risk Level/PW Turned in: 32
...etc

What is the best way to set up the query for this report?
 
Best advice I have for you is to choose a naming convention that does not allow spaces or special characters in field and object names. It will save you a ton of hurt from syntax errors.

If you want more focused advice/assistance, then I suggest you tell us a bit about you and your expertise with databases and access. And tell us in plain English about WHAT your proposed database is about. All simple English, no jargon, no guessing - just straight talk.

Usually a jpg of your tables and relationships, along with any query SQL will help with putting your info into context.

Thanks JDdraw. There really isn't spaces in the field names. I guess I just typed them that way on here for some reason. I am actually ending this thread now since I ended up figuring it out on my own. Again I appreciate you getting back to me!
