Query resets blank after it is run from a form

This used to work properly but am now having issues & can't figure out what changed. I have a query that gets generated when a date range is selected & a status is selected. When the report is created it only uses the search criteria of the Status & disregards the date range selected, then when I open the query up all fields are no longer selected & the parameters are removed. I can't figure out why. See the attachment

A copy of the DB would make it a lot easier to diagnose your problem.
