Query or Form not working properly

I have a query that runs on two parameters from form. If i run the query alone it takes the two parameters and gives me the correct date

But when I run the query through the form it keeps asking me the parameters. Even if I type them in it doesn't take them and gi gives me the wrong data.

HELP????

This may be obvious but all I can think of at the moment: Are you sure that the Record Source for the form is the correct query? I have had 2 queries that were similar in name and function and accidently assigne the form to the wrong one?
Just a stab in the dark!

You need to set the parameters before executing the query. See this post for a DAO solution.
