Records to be displayed when month is selected:--ON THE FORM

Hi Sir,,,,
I have Table name TblBooking containing fileds Chqamt, GST, IT, Rent, Code_Head, FINANCIAL_YEAR, SECTION
Is it possible to display code_head wise records on section of month , fINANCIAL yAER AND SECTION OF DEPARTMENT

like
Code Head SOMETHING
Chqamt 5000
GST 200
IT 200
RENT 1000

I'm unclear what "code_head wise" means.
However it is certainly possible to filter your form data by month or financial year and department.
