Relationships Affecting Report

I need help

I am stuck!!
It's been a while since I've been using Access and I'm trying to merge two data sources into a report.

I can not get my output correct and I'm 99% sure it's my relationships.
Is there anyone that would take a quick look at my file and tell me what I need to do.
I'm sure it's really easy to correct and would not take much time/

Give us something to look at. possibly a scaled down version with just the tables/reports we need to look at (minimal data).
