Report Filter For Form Not Working

I have tried SO hard to see what is wrong with what I did, it worked fine the first DB I built, I cant --- please help -- I will try and upload it again to help see the problem well my zip file is too big -- anyon e I could email it to? Please help me Ill help back in any way I can -- My office expected this last week 000 all looked great until I printed the report
