search records on some parameters

Hi All.
I would like to create Search form. That form will have 3 textbox (Insur#, FirstName, LastName), Search button and listbox. When user will like to find some records then he/she will call Search form type searching data in textboxes, click button and listbox will display information based on entered data. If it is possible to how to create such form?
