Sorting records in a form

I created a main form and want to keep the patient records together. (Some of the patients are entered more than once). Their IDS are the same but their admission dates are different. When I sort the forms I come across several records with the last name "Smith" with first names Jane and Tom. How do I keep all of the records for Jane together and all of the records for Tom together?

My fields consist of Patient ID, Last Name, First Name and Admission Date.

Presumably your form is based on a query. In the query you need to sort by LastName, and then FirstName.
If your form isn't based on a query, in the form design you can use the OrderBy facaility. Type in there LastName;FirstName.
