Subform Relationship Issues

I'm creating a form in Access 2007 that evaluates employee performance. In it is a subform that stores "comments" about the employee as suggestions for improvement. The subform is linked to a table that is separate from the main table. Here's my plan:

1. Each new record shows a blank form that I can input the employee name, their ID and other information linked to them.
2. I also input the month that this information is reported in and their monthly score.
3. In the subform, I have comments to enter. Rather than place 4 separate fields in my main form, I'd like to use a subform so that I can enter as many comments as I'd like.

Everything "looks" like it works, but when I go back to a previous record, the subform entries are not linked to the record I entered them in. They are captured but as a list. I need the comments I enter in an employees monthly form to stay connected to that form so that when I go back through the records, the subform data matches the employees record for that month.

I hope that makes sense.

I have it linked now, but I can't enter more than one line or I get a message that it would create a duplicate entry. I think I have something linked wrong. I want to be able to add several items in this list all under one Rep's name.
