Julie, Why do you have separate tables for referrals and cases? Isn't there only one referral for every case and one case for every referral? You could separate the two in queries, forms and reports, but I don't think it is necessary with tables. OR CAN THERE BE MORE THAN ONE CASE PER REFERRAL...

Rundog, I hope this will help you. I made the ranges of numbers a second table, called tblGroup and set up a relationship (with referential integrity) with tblItem. There are actually SQL statements, not queries, but not obviously visible. You can find a pair of them in each of the Row Sources...

Just add necessary fields to the table the query and the listbox, then you've got it. You can also move and resize the buttons however you want. Note I've referred each button click to the same function.
