What I did is, I separately declared the data values as fields and selected the type for each field as "Yes/No" checkbox, then when I make a form I get these data values as check boxes.

Is this relationship correct? Tables are normalized now?

I intentionally didn't used the field type "InicdentType" because I wanted to have "CheckBoxes" for each type of accident. Secondly, I used the primary key as Accident ID, and then related all table ID fields with that "Accident ID", so that for Accident ID#1 I can have all the other information
