Active X question - Calendar option

everything I have read I can not find a way to have a calendar point to multi fields.

In the Control Source it only will let me point it one table.

I have several date inputs that I would like only one calendar to be able to point to the options

Example

Service_________Check Box-------------Date
xx1_____________Yes/No_____________Date Format
xx2_____________Yes/No_____________Date Format
xx3_____________Yes/No_____________Date Format
etc...
