Auto fill form

I have a list of procedures for them to choose from. What I would like to do is for them to file out a form (time, outcome, comments) as follows

Time Procedure Outcome Comments
Walk the dog
Take out trash
Do homework

They will fill out the time, outcome and comments and when they move to the next page it will record these in all four fields in the table.

I have multiple pages of procedures for them to complete.

I can get it to do this by using a button, but if they press the button again it duplicates the data.

Can some one help?
 
RE,

In your table's design view, set a unique index on the time
and procedure fields. That is the two of them together.
Then Access will take care of it.
