Blank Field In Report Caused By DLookUp In Form

I have seen the solution posted as creating queries, but I am fairly new. I have gotten stuck on this with a couple of reports now. I will attach a small one and if someone could explain what I need to do to fix it I would greatly appreciate it.

You don't need a DLookup, you just need to use a QUERY for your record source instead of just the table. I modified the report you had in there to show you how to do it.
