button Collects Emailaddresses then open a form to create a report

I have a button that collects all the email addresses that have a check mark next to them.
What I want to happen next is trigger a form to open, so I can select from a list of options (this is already built) which in turns runs a report which ultimately gets attached to my above email?? anyone...anyone ever done this??
