Call Flow Idea

Hi Guys,

I'm trying to work out/research the best way to make a simple call flow page for someone using an access database for customer orders. What I want to do is have the database open as usual, but have a form that displays a 'script' of what to say during the call. At the bottom you would have option buttions (yes/no/unsure) which in turn would take you to another page depending on the button selected. What is the simplest way to do this do you think, and is there a general direction you can point me in to research this a little more...
