Combo Boxes on a form and then print Report.

I made a form with several combo boxes on it and I need to print a report
from this form.

I did this in a hodge-pod way and I almost have it working but have hit a wall
on how to fix my problem.

Right now everything is on the report except for the Item Number.
I am sure now that I have done this whole thing wrong.

I have attached a sample of my form and database so you can see what I need
to do and hopefully you can help me out.

I am not opposed to doing it over so I can do it right; so tear my db up if you need too.

I figured it out.
I have attached my working version for those who need to see how I did it.

Might not be perfect but it does the job.
