Creating a Basic Registration Database

Hi

I have created databases (very basic) before but not I need help creating a reliable database for one off events.
Ive looked at software, but this doesnt need to be that advance. Ill try and highlight what Im trying to do, if some has some spare time and could help me out it would be very much appreciated.

The fields that are required are....
entry no./name/email/(favorite colour option in check boxes, multiple options need) BLUE-RED-YELLOW-GREEN... then a submit button which moves to the next blank page (is it possible when you click the submit button it prints off this entry).
Then at any given time I can open excell and I will have a file called BLUE which shows all the names of the people who have had BLUE checked.

In terms of a frontend system what is the easiest to use. The one in access will work fine, but having it load in a web-browser would be good.

Can anyone please help me??

ok.. built the basic database which seems to work fine...

what I do need help with is assigning values to each check box... if the box is checked (value10) and then on the database 'Form' where these details are entered shows the added values!
