Creating a Case Statement

I am currently trying to create a case statement to select one of 5 different fields depending on the result of another field from another table.

I am inexperienced at VB but have reasonable Access skills so any help would be greatly appreciated.

Case statements are easy to use. Read up on it as much as possible and look for as many examples as possible.
