Custom MS Outlook forms?

Hey all,

Just wondering if anyone on here has experience of creating their own MS Outlook forms? I'm basically after 2 answers...

1) I'd like to complete a form and send it. When the recipient opens the email, I'd like them to see the form straight away (not the form as an attachment). Is this possible?

2) Are IF formulas available for easy use in Outlook forms?
