custom search form. help plzz

i want to make a form for a custom search depending on the follwing search terms
name work in nationality catagoria
all info is available in main table "enero"

Although i am a novice access user, i managed to prepare a table, query and search form using like[forms]![formname]![fieldname]&"*".

I need your advice on modifying the search form, i went through the sample database you provided, but since iam not acquainted with VBA, couldn't grasp much. Can you provide a sample database which looks professional as in the previous database, but without the mouse-over effects and list box.

btw the table i created has only 3 fields, i.e Id, keyword and Description.
When i input the keyword into search field it displays the description.
