Data Entry Form User Permissions

Good morning all, I have a quick question regarding my data entry form in my Database. I created the form so that Users would be able to submit software requests via the db. However, it seems I'm the only one who is able to use it. Obviously being admin I should be, however, I've adjusted their single user and group level perms all the way up to admin and they still get a blank form. Does anybody know how I can fix this so that the users can utilize this form?
