Date expire alerter???

I need to create a simple database for myself that should contain name, lasta date check, next time check (which is usually 2-3 years period) and that will alert me 15 days before i need to check name next time! Can anyone help me to create that remindner? It would be the best if the process would run it self in the backround and remind me when I turn computer on.
