Edit/Add record in form

Sorry to beat a dead horse but I have looked around and tried several ideas in other posts with no avail. I have a form that displays records by a unique field. I need to have the user input the identifier if it is already been input the record displays for editing, If it is a new identifier the form is in an add record mode for the user to input the new info for that record. I am using Access 2000.Thanks ahead of time for the help.
 
I have tried combo boxes in every way that I can but it just does not work, sometimes it brings up the wrong record one way I can get it to bring up the right record but if I enter a new identifier that has not been input before it just brings up the very first record for editing. Thanks again for the help

I tried

I did, the combo box never brings up the right record if I choose the look up value in table or qurey, If I choose find record on my form it pulls up the right record but if a new number is put in the combo box it just shows the first record. I need it to add the new number and record if it does not allready exist. My form is frmMain_Input my field is 350 and my combo box is search. Thanks for the help
 
Triple check yourself in where your fields are pointing to in Properties. Sounds like you are pointing to the wrong thing or maybe an old table.
