entering paramater values

I'm trying to setup a user to where all she has to do is enter the beginning date and ending date and then enter a member number ( this works !)
What I would to happen is for her to be able to enter mulitple memeber numbers.
How do I do this???

Could you be a bit more specific about context? Does the range of dates somehow interact with whatever member number(s) get entered, e.g. showing records for these members between these dates? Practically speaking, how many member numbers would typically get entered? Would they get entered at the same time, e.g. one set of dates and several member numbers, or first one set of dates and one member number, then another set of dates and another member number?
